First Circle Medical Center is a nonprofit community clinic serving South Florida (Broward, Miami-Dade & Palm Beach Counties). Our mission is to provide accessible healthcare services including primary care, mental health support, and infectious disease prevention.
Operated by The Snyder Foundation Inc., a registered 501(c)(3) nonprofit organization, our clinic is dedicated to improving community health through compassionate care, prevention programs, and outreach services.
We work to ensure individuals and families in our community have access to the healthcare and support they need to live healthier lives.

Founder & President
Jayne Snyder
The Snyder Foundation Inc. d/b/a First Circle Medical Center
Jayne Snyder is the Founder and President of The Snyder Foundation Inc., a nonprofit organization committed to expanding access to compassionate, high-quality healthcare in underserved communities.
With over 15 years of experience in healthcare operations, Jayne founded First Circle Medical Center with a clear vision: to create a safe, welcoming, and judgment-free environment where individuals can receive the care they need - especially those who may feel overlooked or hesitant to seek help.
Guided by her faith, Jayne is deeply passionate about serving the recovery community and individuals facing difficult life circumstances. Her mission is to meet people where they are, offering support, dignity, and access to essential services including primary care, mental health support, and preventive programs.
Founding Physician
Dr. Jeffrey H. Oppenheimer, MD, MS
Board-Certified Physician · 30+ Years in Medicine
Dr. Jeffrey H. Oppenheimer serves as Founding Physician of First Circle Medical Center, providing clinical leadership and physician oversight in support of the organization's mission to improve access to high-quality healthcare for underserved communities. With more than 30 years in medicine, he is a highly respected physician known for his dedication to patient care, clinical excellence, and professional integrity.
He earned his Bachelor of Arts in Neurobiology and Behavior from Cornell University, a Master of Science in Physiology and Biophysics from Georgetown University, and his medical degree from Georgetown University School of Medicine. He completed his neurosurgical training at the University of Southern California / Los Angeles County Hospital, with a background spanning complex medical conditions, neurological disorders, and chronic pain management.
As Founding Physician, Dr. Oppenheimer oversees clinical quality initiatives, supports chronic disease management programs, and helps guide integrated services that address the physical, behavioral, and social needs of patients. His compassion and commitment to community-based care align closely with the mission of The Snyder Foundation.

Lori Snyder Budihas
U.S. Army Veteran & Certified Medical Billing Specialist
Lori Snyder Budihas is a dedicated healthcare professional and proud United States Army veteran, bringing discipline, integrity, and a strong work ethic to everything she does. With over 10 years of experience in the healthcare industry, Lori has developed extensive expertise in medical billing and coding.
As a co-founder of The Snyder Foundation, she is deeply committed to expanding access to quality healthcare services and making a meaningful impact within the community. Her background in the military has shaped her leadership style and commitment to excellence.

Maria Snyder Prieto is a seasoned healthcare professional with over 20 years of experience in the medical field, bringing a strong foundation in both clinical operations and administrative leadership.
Maria is the co-founder of a successful medical billing company and serves as the Administrator for Optimize Health, where she oversees daily operations and ensures high-quality patient and provider experiences. She is deeply committed to excellence, accountability, and continuous improvement within healthcare systems.

Dr. Jeffrey H. Oppenheimer, MD, MS
Board-Certified Physician · 30+ Years in Medicine
Dr. Oppenheimer brings more than 30 years of medical experience to First Circle Medical Center, where he provides clinical oversight and supports the center's chronic disease management programs. His background spans complex medical conditions, neurological disorders, and chronic pain management.
He earned his medical degree from Georgetown University School of Medicine and completed his neurosurgical training at the University of Southern California / Los Angeles County Hospital. His commitment to community-based care guides the center's clinical quality and integrated services.

Dr. Jeffrey Baker, DO
Board-Certified Family Medicine Physician
Dr. Jeffrey Baker is a Board-Certified Family Medicine physician committed to delivering compassionate, patient-centered care with a focus on improving access to healthcare for underserved and vulnerable populations.
A graduate of Nova Southeastern University College of Osteopathic Medicine, he brings a strong clinical background in primary care, musculoskeletal health, and preventative medicine. His approach emphasizes treating the whole person, addressing not only physical health but also overall well-being and quality of life.

Family & Psychiatric Nurse Practitioner
Herve Jean-Louis, MSN, FNP-C, PMHNP-C
Dual Board-Certified Nurse Practitioner
Herve Jean-Louis is a dual board-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner dedicated to comprehensive, patient-centered care that addresses both physical and mental health. With more than a decade of healthcare experience, he integrates primary care, preventive health, chronic disease management, and behavioral health to improve patient outcomes.
He earned his Bachelor of Science in Nursing from Broward College and his Master of Science in Nursing, Family Nurse Practitioner, from Walden University, later completing his PMHNP certification. His work caring for veterans in the psychiatric unit of the Miami VA Healthcare System deepened his passion for mental health care, trauma-informed treatment, and suicide prevention.
At First Circle Medical Center, Mr. Jean-Louis provides primary care and behavioral health services with a whole-person philosophy that recognizes the vital connection between mental and physical health. His clinical interests include anxiety and mood disorders, crisis intervention, and expanding access to quality mental health care for underserved populations.
Family Nurse Practitioner
Theresa Charlotin, FNP-BC
Primary Care & Functional Medicine
Theresa Charlotin is a board-certified Family Nurse Practitioner with a diverse clinical background spanning primary care, functional medicine, addiction therapy, IV hydration, aesthetics, and wound care. She is known for her patient-centered approach, strong clinical judgment, and commitment to evidence-based practice.
She provides comprehensive care to adult and geriatric patients, with experience in preventive health, chronic disease management, acute care assessment, and patient education. She has a particular interest in improving access to care through concierge-style services and community-based health initiatives.
Ms. Charlotin also serves as a nursing professor, mentoring the next generation of nurses, and contributes to multidisciplinary addiction treatment teams with compassionate, nonjudgmental care focused on patient safety and long-term recovery.
Licensed Mental Health Counselor
Dr. Sunny Khachatryan, Ph.D, LMHC, CAP
Addiction & Mental Health Treatment
Dr. Sunny Khachatryan is a Licensed Marriage and Family Therapist who has helped individuals, couples, and families achieve meaningful, lasting change since 2015. She earned her Bachelor's in Psychology from San Diego State University, a Master's from Barry University in Mental Health Counseling and Marriage, Couples, and Family Counseling, and her PhD in Family Therapy from Nova Southeastern University.
Her approach is collaborative, relational, and strengths-based. She specializes in addiction and mental health treatment, working with clients facing anxiety, depression, trauma and PTSD, mood and substance use disorders, relationship and family challenges, postpartum depression, and life transitions. She also has extensive experience providing culturally sensitive care to Native American communities, including members of the Seminole Tribe of Florida.
At the heart of her work is the belief that healing begins with self-love and self-compassion. She offers a warm, supportive, and nonjudgmental environment where clients can explore challenges, build resilience, and develop a deeper sense of self-acceptance.
In honor of Rudi Johnson #32. Dr. Khachatryan dedicates her work to raising awareness of Chronic Traumatic Encephalopathy (CTE), mental health, and the importance of seeking support.
