What to have ready
Having these on hand makes the intake form quick. If you do not have all of them, start anyway and we will help you finish.
Photo ID
A driver's license or other photo identification. You can upload a picture of it.
Insurance card
Medicaid, Medicare, or your private plan. Uninsured patients are welcome too.
Reason for your visit
A short description of what you would like to be seen for.
