Who Qualifies
The sliding fee discount is based on your family size and household income, measured against the federal poverty guidelines. If your income falls within the program's guidelines, your fees are adjusted to a level you can afford, and no one is turned away from care because they cannot pay.
How to Request an Application
Requesting an application is simple. Call the front desk at 954-663-2255 and ask for a Sliding Fee Discount Program application, or ask for one in person at your next visit. Our team will walk you through what documentation is needed and help you complete it.
